Your Path to Becoming a Paid Caregiver

To qualify for the program, your loved one must have active Michigan Medicaid. If they don't, they'll need to apply first at michigan.gov/mibridges.

Their doctor needs to fill out a form (called the DHS-54A) that confirms they need help with daily tasks like bathing, dressing, or cooking. We can guide you on how to request this.

You'll complete a caregiver form (the DHS-390) that lists who will be helping. A family member, friend, or roommate may qualify to get paid. We'll help you fill it out.

The State will send a caseworker (an Adult Services Worker) to the home. They'll review what kind of help is needed and approve the number of care hours.

We take care of the final step: your official caregiver registration in Michigan's system (called CHAMPS). Once approved, you can start getting paid for the hours you provide.
